A risk assessment form template is eight columns and three blocks, and the template on this site prints it from your task's figures rather than handing over a blank grid. The columns, one row per hazard: hazard, who is at risk, how, likelihood, severity, control, residual risk, further action. The blocks: the header with task, site, author, reviewer and dates; the significant findings; the footer with signatures and the review date. The HSE's guidance, cited below, gives risk assessment as five steps: identify hazards, assess the risks, control the risks, record your findings, review the controls, and says that if you employ 5 or more people you must record your significant findings. The columns
Each hazard numbered so the method can reference it; each column filled, because an empty residual risk or action is the row an auditor reads as unfinished. Each column is filled for every hazard row, so a blank cell is a question the approver will ask.
The header
Task in a sentence, site, job reference, author, reviewer, version, date. The header files the form and names who is accountable. The header is what the file is found by later, so the job reference and the date go in before anything else is written, and the revision number moves with every change.
The findings and the footer
The significant findings regulation 3, cited below, names; the author's and reviewer's signatures; the review date and the trigger events. The footer carries the assessor's name and signature, the date, and the review date the assessment is next read against the site.
Kept
In its version against the job with the method built from it. DailyLogDesk Pro keeps it; the template prints it. From the vendors' own pricing pages, cited below: SafetyCulture lists its Premium plan at $29 per seat per month billed monthly or $24 billed annually, with a free plan for small teams; Contractor Foreman lists its Basic plan at $49 per month for one user. DailyLogDesk Pro is $29 per month for the whole site team.
