A blank risk assessment form, and why this site prints the form already filled from your figures

A blank risk assessment form is the usual search, and the usual problem: the form is downloaded, the columns are right, and it is filled at a desk from memory or not at all. The risk assessment template on this site prints the same form from your task's own hazards, people at risk, controls and residual risks, so what you print is the record. Regulation 3 of the Management of Health and Safety at Work Regulations 1999, cited below, requires every employer to make a suitable and sufficient assessment of the risks to the health and safety of their employees, and regulation 3(6) requires an employer with five or more employees to record the significant findings and any group of employees especially at risk. Free, no account; the paid product keeps the printed form against the job.

The columns a blank form should have

Hazard, who is at risk and how, existing control, residual risk, further action with an owner and a date. Plus the header: task, site, date, author, reviewer, review date. And the block most blank forms omit: the significant findings. The form on this site has all of them.

Why filled beats blank

The site-specific hazards are known on site, at the walk-round, and a form filled there is the assessment; a form filled later is a recollection. Entering the figures into the tool on a phone at the site prints the assessment before the van leaves. The HSE's five steps, cited below, end in record, and the record is made when the hazards are in front of you.

What stays yours

The hazards, the controls, the judgement of likelihood and severity. The tool sizes, formats and prints; it does not supply a hazard, because a supplied hazard is a generic one, and a generic hazard is the one an approver returns.
