A point of work risk assessment template: the five questions asked at the task before it starts, on a card, with the RAMS behind them

A point of work risk assessment template is not another RAMS; it is a card the operative completes at the task, on the day, before starting, to check the RAMS still fits. Five questions: has anything changed since the RAMS was written; are the controls it names in place; is the plant inspected and the PPE right; is the area safe, including who else is in it; is there anything new. Signed, and the task stopped if any answer is no. The risk assessment template on this site prints the RAMS the card checks against, free and without an account.

The five questions

Changed, in place, plant and PPE, area, new. Each a yes or a no with a line for what. A card with an essay box is not completed at seven in the morning; five ticks and a line are. The card is printed with the RAMS number on it, so the operative checks against the right document.

What a no means

The task does not start until the foreman has decided: the control put in place, the RAMS revised and re-briefed, or the task deferred. The no is recorded with the decision. The HSE's guidance, cited below, gives risk assessment as five steps: identify hazards, assess the risks, control the risks, record your findings, review the controls, and says that if you employ 5 or more people you must record your significant findings.

What the card is not

It is not the assessment; the RAMS is. The card is the check that the assessment still describes the site, and it references the RAMS by number so the two are read together. A card without a RAMS behind it is a dynamic assessment with nothing to be dynamic about.

Kept

Against the job with the RAMS and the daily log, so a month of cards shows how often the site changed. DailyLogDesk Pro keeps them; the template prints the RAMS. From the vendors' own pricing pages, cited below: SafetyCulture lists its Premium plan at $29 per seat per month billed monthly or $24 billed annually, with a free plan for small teams; Contractor Foreman lists its Basic plan at $49 per month for one user. DailyLogDesk Pro is $29 per month for the whole site team.
