A simple risk assessment template: one page for one task, with the record the law names still on it

A simple risk assessment template fits on a page when it covers one task and the hazards that matter for it, and it is still the record the law describes if it carries the significant findings, the author, the date and the review. The HSE's guidance, cited below, gives risk assessment as five steps: identify hazards, assess the risks, control the risks, record your findings, review the controls, and says that if you employ 5 or more people you must record your significant findings. The risk assessment template on this site prints the one-page shape from the task's hazards, free and without an account; the paid product keeps the page against the job.

One task, one page

The task in a sentence at the top, the site, the date. Then the hazards of this task, up to about six; a task with more is usually two tasks or needs the fuller template. Each hazard on one line: who is at risk, the control, the residual risk.

The controls are specific

Simple does not mean vague. Edge protection, isolation proved dead, exclusion zone, dust extraction, a banksman: named measures. The word care in a control column is the sign the assessment has not been done.

The record on the page

Regulation 3 of the Management of Health and Safety at Work Regulations 1999, cited below, requires every employer to make a suitable and sufficient assessment of the risks to the health and safety of their employees, and regulation 3(6) requires an employer with five or more employees to record the significant findings and any group of employees especially at risk. The significant findings are two or three lines under the rows; the author, date, reviewer and review date are in the header. That is the whole of the legal record for a simple task.

Where it goes

Against the job, with the method statement if the site wants one and the sign-on if the crew is briefed on it. DailyLogDesk Pro keeps it there; the template on this site prints it. From the vendors' own pricing pages, cited below: SafetyCulture lists its Premium plan at $29 per seat per month billed monthly or $24 billed annually, with a free plan for small teams; Contractor Foreman lists its Basic plan at $49 per month for one user. DailyLogDesk Pro is $29 per month for the whole site team.
