A RAMS document template is seven blocks in a fixed order, and the order serves three readers: the approver reads the header, the assessment and the emergency block; the crew reads the method; the inspector, later, reads the sign-on and the review. Regulation 3 of the Management of Health and Safety at Work Regulations 1999, cited below, requires every employer to make a suitable and sufficient assessment of the risks to the health and safety of their employees, and regulation 3(6) requires an employer with five or more employees to record the significant findings and any group of employees especially at risk. The risk assessment and method statement tool on this site prints the seven blocks from your task's figures, free and without an account.
Blocks one and two: header and task
Contractor, client, site, job reference, dates, people, author, reviewer, version. Then the task in a sentence and its location on site. The header is what files the document; the task sentence is what the approver checks the rest against.
Block three: the risk assessment
One row per site-specific hazard: who is at risk, how, likelihood, severity, control, residual risk, and the significant findings stated as a block. The HSE's guidance, cited below, gives risk assessment as five steps: identify hazards, assess the risks, control the risks, record your findings, review the controls, and says that if you employ 5 or more people you must record your significant findings.
Blocks four and five: method, and plant, permits, PPE
The steps in order with the assessment's control at the step it applies; then the plant with its inspection status, the permits the task needs, and the PPE per step. The crew reads these two blocks at the briefing, so they are lists rather than prose.
Blocks six and seven: emergency, and sign-on and review
First aid, hospital, the site's emergency procedure, the rescue plan for height. Then the crew's signatures with the date, and the review date with the trigger events. DailyLogDesk Pro keeps the signed document per job and reminds the contractor at the review; the tool prints the template. From the vendors' own pricing pages, cited below: SafetyCulture lists its Premium plan at $29 per seat per month billed monthly or $24 billed annually, with a free plan for small teams; Contractor Foreman lists its Basic plan at $49 per month for one user. DailyLogDesk Pro is $29 per month for the whole site team.
