Completed risk assessment examples for three tasks, a roof repair, a trench and hot works, with every column filled so the finished shape is visible

Completed risk assessment examples are worth more than blank templates because the blank never shows what a finished row looks like. Three here, each summarised: a roof repair from a tower, a hand-dug service trench, and hot works in an occupied building. Every row with every column, the significant findings, the footer. The HSE's guidance, cited below, gives risk assessment as five steps: identify hazards, assess the risks, control the risks, record your findings, review the controls, and says that if you employ 5 or more people you must record your significant findings. The risk assessment template on this site prints the same completed shape from your own task, free and without an account.

The roof repair

Five rows: the fall, the tower, falling materials, manual handling, the fragile skylight. Controls: guardrails, the tower tied and inspected, the harness, the exclusion zone and banksman, split loads, crawling boards. Significant findings: the fall and the public. Rescue plan in the method.

The trench

Four rows: the services, the collapse, the edge loading, the public. Controls: the scan and hand digging, support beyond the stated depth, spoil back from the edge, barriers and a covered walkway. Significant findings: the services and the collapse.

The hot works

Four rows: the fire, the fumes, the cylinders, the occupants. Controls: the permit, the clearance, the fire watch, extraction, the cylinders secured with arrestors under the DSEAR row, the occupants informed and the detectors managed. Significant findings: the fire and the cylinders.

What the three share

Every column filled, the findings written, author and reviewer separate, the review date set, the controls placed in the method. Regulation 3 of the Management of Health and Safety at Work Regulations 1999, cited below, requires every employer to make a suitable and sufficient assessment of the risks to the health and safety of their employees, and regulation 3(6) requires an employer with five or more employees to record the significant findings and any group of employees especially at risk. DailyLogDesk Pro keeps the assessments; the template prints them. From the vendors' own pricing pages, cited below: SafetyCulture lists its Premium plan at $29 per seat per month billed monthly or $24 billed annually, with a free plan for small teams; Contractor Foreman lists its Basic plan at $49 per month for one user. DailyLogDesk Pro is $29 per month for the whole site team.
