Every figure on this page is computed from the inputs you enter, by the method stated below it. The only constants DailyLogDesk supplies are the published reporting clocks and assessment steps, each cited where it is used. The defaults are a worked example to replace with your own.
Risk assessment forms for a small contractor, in the one shape an approver can read at a glance: a header for the task, the site, the date and the revision; numbered hazard rows with who is harmed, the likelihood and severity before and after the controls, the control and the person responsible; a findings block; and a signed footer with the review date. The risk assessment template on this site prints the form filled from your task rather than as a blank grid, free and without an account; DailyLogDesk Pro keeps the forms against each project with the review reminder.

The header: what the form is found by later
The task, the site, the client, the date, the assessor and the revision number. The header is what the file is searched by when an inspector, a client or an insurer asks for the assessment, so it is filled before any row is written and the revision moves with every change.
The rows: eight columns, every cell filled
The hazard, who is harmed and how, likelihood and severity before the controls, the control, likelihood and severity after it, and the person responsible. A blank cell is a question the approver will ask; the template on this site fills every column from your inputs and scores out of 25.
The findings and the footer
The significant findings summarised, the groups especially at risk, the further action with a name and a date; then the assessor's signature, the date and the review date. That is the block regulation 3 asks for from an employer with five or more employees, and the one most forms leave half filled.
Filed with the method, the briefing and the log
The form is one part of the job's record: the method statement applies its controls, the toolbox talk briefs them and the daily log records the checks. Pro keeps them together against the project and exports the set when it is asked for.

Risk assessment forms: common questions
Are the risk assessment forms free?
Yes. The risk assessment template on this site prints the filled form with no account and no card. Pro keeps the forms against the project and raises the review.
Is there one form for every task?
One shape; the rows change with the task. The columns are the same for a roofer and an electrician because the approver reads them the same way.
Can I fill the form on a phone?
Yes. The template works on a phone at the site, which is where the form should be filled, and Pro saves it against the project from there.
