A CDM template means two different documents depending on who is asking. The Construction (Design and Management) Regulations 2015, cited below, require a construction phase plan, drawn up by the principal contractor, or by the contractor where there is only one, and regulation 12 is where the plan and the health and safety file sit. A small contractor working under a principal contractor does not write the plan; they supply the RAMS for each task that the plan is checked against. The risk assessment and method statement tool on this site prints that RAMS, free.
The construction phase plan
The arrangements for managing the health and safety of the construction phase: the site rules, the organisation, the significant risks and how they are controlled, the emergency procedures, the welfare, the arrangements for controlling specific risks. It is the principal contractor's document, proportionate to the project, and it exists before the site opens.
What a contractor supplies under it
For each task, a RAMS: the task-specific assessment and method, consistent with the plan's site rules and its arrangements for the risk in question. The principal contractor approves it against the plan. The HSE's guidance, cited below, gives risk assessment as five steps: identify hazards, assess the risks, control the risks, record your findings, review the controls, and says that if you employ 5 or more people you must record your significant findings.
The one-contractor project
Where there is only one contractor, that contractor draws up the plan themselves, proportionate to the job, and their RAMS for each task sit under it. A small domestic extension with one builder still has a plan; it is short. The tool on this site prints the RAMS; the plan is the contractor's own document.
The health and safety file
For projects with more than one contractor, the principal designer prepares the file of information the client will need for future work, and the contractor's records feed it. DailyLogDesk Pro keeps the RAMS, daily reports and incident reports against the job, which is the record the file draws on. From the vendors' own pricing pages, cited below: SafetyCulture lists its Premium plan at $29 per seat per month billed monthly or $24 billed annually, with a free plan for small teams; Contractor Foreman lists its Basic plan at $49 per month for one user. DailyLogDesk Pro is $29 per month for the whole site team.
